Welcome to the 2011 – 2012 Children’s Faith Formation (CFF) program. The complete list of Registration materials for the upcoming academic year is listed below. When you clck on the link a new pdf page will open. Please print the page, fill it and bring it to the Parish Office or scan it and send it via email to Children's Faith Formation Director.

Change of Address/Phone – Please notify the CFF office of any change of address or telephone number immediately so that the catechetical staff will be able to contact you in the event of an emergency involving your child.
Custody Arrangements – The primary responsibility for seeing that custody arrangements are carried out rests with the parents. The catechetical staff wishes to cooperate with special custody arrangements of families to the best of their ability. Parents are asked to meet with and submit in writing detailed information regarding such arrangements to the CFF coordinator.
